How to Make a Contribution by Mail

1. Download a contribution form. There are two options for your donations.
   Annual Fund Contribution (download form as .PDF) (used for scholarships, programs, and other unrestricted areas of need)
   Endowment Fund Contribution (download form as .PDF) (used for scholarships, programs, and specific endowment funds as designated)
2. Complete the form and submit with a check payable to: "Charter Oak State College Foundation".
3. Mail, e-mail or fax to the Foundation with the details of your gift or pledge.
